As part of the TMF Home Health Quality Improvement initiative, TMF provides Texas agencies with tools, resources and evidence-based practices to prevent avoidable hospitalizations and improve patient self-management of oral medications. As part of this initiative TMF partners with home health professionals to examine their practices and improve systems of providing care. We provide free education to participating agencies based on proven best practices to help ensure that care is safe, effective, timely, patient-centered, efficient and equitable.
